6th Grade Outdoor Education Information
DATE
Check school calendar
The day we leave.
Parents bring students to school at the regular time (students can’t ride bus with bags)
put all bags, with labels, by room 102 (gym if raining); go to 1st period
we will board bus between 8:45-9:15.
The day we return.
We return between 2:30-3:00; parents must pick up students; no riding bus with bags;  students may leave after we return and getting their bags.  No need to wait until 3:05.
FORMS
An information/permission form
(will be provided) must be filled out prior to any student going to outdoor education week. All forms will be sent home with students   
Medication form
(will be provided) for any and all medications, including non-prescription medicines or vitamins, must be filled out completely with doctor’s signature. Exact times and days that meds need to be taken, exact dosage, and medication names must be exactly the same on the form as on the packaging.  All medications must be in original packaging.  Staff cannot administer incorrect medications or medications without doctor’s approval on form. 
Forms and medications must be placed in a plastic zip-lock bag, labeled with student’s name.

Packing information
list will be sent home to parents  NO GUM, FOOD, CANDY ALLOWED.
Food information
Food is served family-style, take what you want but eat what you take.  Special diets (lactose-intolerant, vegetarian, etc.) can be accommodated but must be stated on health form.
Cost
It may be paid in full, by selling the fundraiser, or a combination of selling and paying the difference. If you decide to sell the fundraiser, do not pay in full. No refunds on fundraiser money earned. First come, first served. Please check with 6th grade teachers for current information
